Efeito do tratamento atraves do uso da placa de Michigan, na evolução da sintomatologia do aparelho estato acustico, em pacientes com sindrome da disfunção muscular e da ATM

RESUMO

Using a full coverage maxillary splint (of Michiigan design), for a period of 8 weeks, as the only treatment for the TMJ myofacial pain dysfunction syndrome, treatment for the TMJ myofacial pain dysfunction syndrome, the clinical responses of the subjective ear symptoms were assesed in a group of 49 patients. Audiometric test were carried out at the beginning of the treatment in ll patients. The splint was inserted and a weekly assessment was carried out for a period of eight weeks at the end of which the data were computed and analised considering 3 periods or phases of assessment (4 inicial, 4 final and 8 complete weeks). The subjective responses of the symptoms were analised statistically by using the T WILCOXON test (p=0,02). I a group of 12 patients in which the treatment continued for different periods of time ranging from eight weeks to one year after the insertion of the splint, a second audiometric assesment was carried out and the two results wers compared. The statistical differences in success rate were considered significant for otagia, tinnitus and vertigo reaponses and non significant for the responses of subjective hearing loss, autophony and stuffly sensation in the ear. However, there was a noticeable improvement in all the ear symptoms, chiefly when the results of the four final weeks (2nd. phasel of the treatment were considered, in which 32,5% or more of the patients improved 80% or above in all symptoms. In the group of 12 patients in which the significant changes in the audiometric conditions of each patient. However, a comoarison of the results of the subjective symptom assessment and the initial audiometric evaluation shower that 55% or more of the patiensts wich related each one of the symptoms and had initial abnormal pure tone treshould audiograms or abnormal impedance fidings, presented subjective improvements in the symptoms iqual to, or above 65%, considering the mean of the four final weeks (2nd. phase) assessment (Figs. 1F and 2F). These findings point to the need for greater attention to the TMJ dysfunction syndrome by the otorynolaryngologic diagnosis and for a grater cooperation between the two fields
