Estudo prospectivo das complicações biliares e fatores de risco associados apos o transplante hepatico ortotopico em adultos

RESUMO
Liver transplantation is the most effective treatment option for patients with end-stage liver disease. Bespite improving results, surgical morbidity is still significant, and often relates to the biliary reconstruction. The aims of this study are to define and compare the incidence of biliary complications after choledococholedocostomy with and without "T" tube and choledococholedocojejunostomy with Roux en Y, and assess the predisposing etiologic factors and the therapeutic options. Two hundred thirty nine orthotopic liver transplantation were performed in 222 patients who were followed up prospectively, at the Liver Unit-Birmingham. They were divided into three groups according to the type of biliary reconstruction: Group 1 - 110 grafts with choledoc?choledocostomy with "T" tube, Group 2 - 89 with choledococholedocostomy without "T" tube, and Group 340 with choledocojejunostomy with Roux en Y. Forty-four grafts (18,4%) presented with biliary complications, 26 (23,6%) in Group 1, 10 (11,2%) in Group 2, and 8 (20%) in Group 3. There was a statistical difference between Groups 1 and 2 (p<0,05). Bile leaks accounted for 59,1% of the cómplications followed by obstruction in 38,6%. The use of "T" tube was the only predisposing factor resulting in increase in complications. However, patients were, treated conservatively in 42,3% in Group 1, versus 20% and 25% in Groups 2 and 3, respectively. The remender required radiological or surgical intervention. Two patients died as a result of their biliary complication, one from Group 1 and other from Group 3. In conclusion, the significant difference between rates of biliary complication in Groups 1 and 2 was due to bile leaks associated with "T" tube remova!. The surgical morbidity after liver transplantation can be decreased by abandoning of "T" tubes with choledococholedocostomy.
